Osteotomy for Class III and Long-Term Results: Indication for 1 or 2 Jaw Surgeries

An anterior and a posterior alveolar osteotomy in a maxilla or the segmentation of a Le Fort I osteotomy is the operative method which is applied when discoordinate the width of maxilla and mandible alveolar arch or a maxillary protrusion. In these cases, a bone fixation after an alveolar osteotomy are mini or micro plates at the labial or buccal side, in addition the dental wiring of resin plate at palatal side are generic method. However, operation of a perioperative correction wire or a pollution of postoperative palatal plate becomes a problem.

I use locking plate system of palatal side in a bone fixation of an alveolar osteotomy. More tophaceous dipping can be performed by changing a bone fixation into dipping by locking plate from palatal plate, and excellent results are obtained.

It is reported about a summary and a case of a fixed mechanism. All cases are anterior alveolar osteotomy, posterior alveolar osteotomy and segmented of Le Fort I osteotomy. As for the trans-mucosal fixation of the palatal side which locking plate was used for, perioperative operation was easy, and without pressure to mucous membrane of palate, the dipping situation of bone segment was excellent. Furthermore, I performed the plate removal in an outpatient department easily, and it was thought with useful methods.
